How to Request a Pre-Adoption Consultation

Dr. Douglass will not be taking new pre-adoption evaluations from June 12th until July 1st. We apologize for the inconvenience.

The heart of the consultation is a 30-60 minute personal phone conference with Dr. Douglass reviewing your child's medical information. Understanding the time pressure and the anxiety surrounding the receipt of your referral we strive to schedule phone conferences within 1 - 2 business days. Dr. Douglass will review your child's information with you in detail from unusual sounding diagnoses to growth patterns. He will also discuss information that you may not have and guide you in formulating questions for your agency. Dr. Douglass is then available by email to analyze any follow-up information you may gather from your agency and/or during your travel.

Steps to request a Pre-Adoption Consult

1. Download and fill out the Personal Information Sheet and the Consent Form and return by email or fax along with electronic versions of your information (photos and translated medical information). If you have a video, overnight mail a COPY. We will not be able to return the video or hard copies of photos. If you know you are going to be getting more information from your agency soon please wait until you have all your information and then send it all together.

2. At the same time please call our office (704-786-1144, M-F 8:00 AM - 5:00 PM EST) and notify us that you are requesting an adoption consultation with Dr. Douglass and have sent your initial paperwork and information. This allows us to make contact if we did not receive your email.

3. Once we receive all your information our staff will call and set up a phone conference. Phone conferences are scheduled for late mornings or afternoons (EST) M-F.
